Maple & Moon Cat Rescue Adoption Agreement
By checking this agreement, I acknowledge that I am adopting the cat listed below from Maple & Moon Cat Rescue. I understand that adoption is a lifelong commitment, and I agree to provide a safe, loving, and permanent home for this cat.
As the adopter, I agree to the following:
1. I will provide this cat with adequate food, fresh water, shelter, enrichment, love, and routine veterinary care throughout its life.
2. I understand that this cat is to be kept as an indoor companion. I will not allow this cat to roam freely outdoors. Outdoor access is permitted only in a secure enclosed catio, enclosed pet stroller, or while wearing a properly fitted harness under direct supervision.
3. I agree that this cat will never be declawed. I understand that declawing is an unnecessary amputation procedure that can cause lifelong physical and behavioral problems. If scratching becomes an issue, I will use humane alternatives such as scratching posts, nail trimming, nail caps, or behavior training.
4. I agree to seek prompt veterinary care if this cat becomes sick or injured and to maintain recommended preventative veterinary care.
5. I understand that this cat is being adopted as a companion animal and will not be used for breeding, research, fighting, or any illegal activity.
6. If, at any point during the cat's lifetime, I am unable or unwilling to keep this cat, I agree to contact Maple & Moon Cat Rescue immediately. I understand that I may not abandon, surrender, sell, trade, give away, or otherwise transfer ownership of this cat without first giving Maple & Moon Cat Rescue the opportunity to have the cat returned to its care or to approve another placement.
7. I understand that Maple & Moon Cat Rescue makes every reasonable effort to evaluate each cat's health and temperament before adoption. However, no guarantees are made regarding future health, behavior, age, or temperament.
8. I understand that once adoption is finalized, I assume responsibility for all future veterinary care, expenses, and daily care of the cat.
9. I agree to update the cat's microchip registration, if applicable, with my current contact information as soon as reasonably possible after adoption.
10. I understand that Maple & Moon Cat Rescue may contact me after adoption to check on the cat's well-being and to offer support if needed.
11. I certify that all information I provided during the adoption process is true and accurate to the best of my knowledge.
